We provide same day appointment for urgent problems as long as the schedule permits. We see all patients by appointment so that you have less wait time in our office (unless we need to accommodate call in patients with acute illness).

Payments

We accept payments by cash/check/credit cards (VISA or MasterCard effective 05/24/2018). We will bill your insurance as a courtesy but you are required to pay your deductibles, copayment and co-insurance at the time of your visit. If you have a large deductible, we request payment as allowed by your insurance or as estimated, especially if your insurance does not provide accurate information about the allowed payments for services charged.

Insurances

We accept Medicare and almost all Private Insurances/PPO/POS/EPO including (but not limited to) Blue Shield, Blue Cross, Aetna, United Healthcare, CIGNA (CIGNA LocalPlus Products as we are CIGNA Care Designated Physician), PHCS, LABORERS, UFCW, Multi-plan, HealthNet, PacifiCare, Great-West, Humana, Pacific Health Alliance, USA Health Network (USA-MCO), and others through Medical Dimensions Inc (Aetna US HealthCare, Beech Street, Coastal HC administrators, Coventry Health, CorVel Corp., First Health Network, Galaxy Health Network, Health MGMT Network, Humana, Interplan, MULTI-PLAN/PHCS, PacificCare/United Healthcare, USA Managed Care Org. (PPO)). We accept HMO insurances through Santa Clara County IPA (SCCIPA). We give a discount to uninsured patient/ "cash account." Dr. Mandakini Patel also accepts HMO insurances through Physicians Medical Group of San Jose (PMG). We participate with most COVEREDCA.COM insurances. We accept Valley Health Plan (Commercial, Covered California, and Individual Plans).

If you have HMO insurance, you must have appropriate Dr. Patel listed as your physician with the insurance and IPA before we can see you. You are required to bring in your CLD/ID and insurance cards on first visit. Please promptly notify our office of any changes in your insurance, address, phone numbers (set up to receive voice messages) and disclose all insurances at the time of your visit.

Visit Requirements

All pediatric patients must be accompanied by their parents/guardians on each visit. If you/your child miss your appointment, you must call and reschedule the visit.

For Cancellation/No Show

A fee of $75 will be charged without 24 hour notification.
